Ordering an insurance card

If you have been waiting for your new insurance card for a long time or have lost your current card, you can get help here.

It may take up to four weeks for you to receive your insurance card. If you have not yet received a card, please check that your personal details, such as your name, surname, date of birth and gender are correct on your insurance policy. Report any errors to our Customer Service team or visit your nearest sales office.

Please state the reason why you need a new card on the form.
